This page is not medical advice. It describes the service and how to arrange it. For a severe episode — a serious asthma attack, chest pain, or symptoms of very high or very low blood sugar such as confusion, fainting or difficulty breathing — call 911 or go to the nearest emergency department. Toronto Western Hospital is directly across the street.
Why continuity matters more than convenience
A walk-in visit treats one problem in isolation. For a long-term condition that is the wrong shape of care: the clinician has no baseline to compare against, no record of what has already been tried, and no way to see a slow trend.
Ongoing management is the opposite. The same physician sees the pattern across visits, adjusts before something becomes urgent, and already knows your history when something does go wrong. That is the argument for enrolling rather than repeat walk-ins — and it is worth more than the convenience you give up.
What a review visit covers
Review
How you have been since the last visit, what has changed, what is not working.
Monitoring
Bloodwork or other testing as needed, and comparison against your own previous results.
Adjustment
Medication changed, added or stopped based on what the monitoring shows.
Referral
To a specialist or for imaging when the condition needs more than primary care — including nearby Toronto Western Hospital services.
Prescriptions on site
The pharmacy at 750 Dundas St W #11is in the same building, so a prescription or refill can be filled in the same trip. It keeps wider hours than the doctor’s practice — Monday to Friday 9:00 am to 7:00 pm, Saturday and Sunday 9:00 am to 3:00 pm — so collection is not tied to appointment hours.
Common questions
What does ongoing chronic disease management involve?
Regular review appointments with the same physician, monitoring through bloodwork and other testing, adjusting medication as needed, and referrals to specialists when appropriate. The point is continuity — one doctor holding your full history rather than starting over each visit.
Which conditions are managed at the clinic?
Dr. Harris Hashim manages diabetes, high blood pressure, asthma and thyroid conditions among other long-term conditions as part of ongoing family practice at 750 Dundas St W.
Do I need to be enrolled as a patient, or can I use walk-in?
Chronic conditions need continuity, so enrolling is strongly preferable to repeat walk-in visits. Walk-in is designed for single immediate concerns and the physician will not have your full history. Dr. Hashim is currently accepting new patients — call (416) 681-0444 to enrol.
Can I get my prescriptions filled at the same visit?
Yes. There is a pharmacy on site at 750 Dundas St W, so new prescriptions and refills can be filled in the same trip. The pharmacy keeps wider hours than the doctor's practice: Monday to Friday 9:00 am to 7:00 pm, and Saturday and Sunday 9:00 am to 3:00 pm.
How often will I need to come in?
That depends on the condition and how stable it is. Some conditions are reviewed a few times a year once settled; others need closer monitoring after a medication change or a new diagnosis. The interval is set with you at each visit rather than fixed in advance.
Can you take over management from my previous doctor?
Bring whatever records you have to your first visit — a medication list, recent bloodwork, or a summary from your previous physician — and ask about transferring your full records. Your care does not have to wait on the transfer arriving.
