Tell the team you would like to register as a new patient with Dr. Hashim. Booking is by phone — there is no online registration form.
Book your first appointment
Your first visit is a longer one. It covers your history, current medications, any ongoing conditions, and what preventive screening you are due for.
Bring your health card and medication list
Ontario health card, photo ID, a list of current medications and doses, and any known allergies. If you have records or a referral letter from a previous doctor, bring those too.
Ask about your records
At that first visit, ask about transferring your full records from a previous family physician. Timing depends on the other practice — your care does not have to wait on it.

Why this is hard to find in Toronto
Finding a family physician taking new patients in downtown Toronto is genuinely difficult — many practices are closed to new patients or keep a waiting list. A practice accepting patients directly is the exception rather than the rule.
It also means capacity is finite. Availability here can close without notice, so if you are looking, call rather than bookmark.
Ontario also runs Health Care Connect, a provincial service that matches people to a family doctor or nurse practitioner. Registering there and calling here are not mutually exclusive.
Meanwhile, walk-in is open
You do not need to be enrolled to be seen. Walk-in care runs Monday to Thursday 9:30 am–12:00 pm and 2:00 pm–4:00 pm, and Friday 9:30 am–12:00 pm and 3:00 pm–4:00 pm, with no appointment and no referral. More on walk-in care.

Do I need to be referred by another doctor?
No referral is required to register with a family physician in Ontario. You can enrol yourself by calling the clinic directly.
What about my medical records from a previous doctor?
Bring whatever you have to your first appointment — a summary, a medication list, or a referral letter. Ask the clinic at that visit about transferring your full records from your previous family physician; the process and timing depend on the other practice. Your care does not have to wait on records arriving.
What is the difference between enrolling and just using the walk-in?
Enrolling means one physician holds your full history, manages chronic conditions over time, orders screening, and handles referrals to specialists. Walk-in visits treat one immediate problem with no ongoing relationship. Both are available at 750 Dundas St W.
Can I register if I do not have an OHIP card yet?
Call and ask. Coverage under UHIP or GuardMe, or while an OHIP application is in progress, is handled case by case — the clinic will tell you what applies before you attend rather than at the desk.
